How to design script in matlab for Simulink model

Hello I have implemented simulink model for LQG controller

but to initialse model and calculate state feedback gain and kalman gain , there are commands.

should i use state space command in matlab or in simulink model i have used state space block. so just used A,B,C,D matrices and about noises , like process noise and measurement noise, how to add these noises in simulink?

any one having example of LQG simulink model with matlab script ?
