Excite Single Input of a MIMO System

I am simulating a system with two states and two control inputs using simulink via a state space model. I have attatched two sources to the state space model and need to specify which source maps to which control input of the state space model.
B is a 2x2 matrix. The plant block diagram should have two inputs, however it only has one.
How do I pass the two unique control inputs to the MIMO state space model? Imagine the plant was an aircraft, and I am attempting to model pitch and yaw control.
How should I modify the below block diagram to specify such?

Solved using a Vector Concatonate block

That actually misses the heart of the question.
Allow me to re-phrase. I am simulating a Multiple-Input-Multiple-Output system; B is a 2x2 matrix. The plant block diagram should have two inputs, however it only has one.
How do I pass the two unique control inputs to the MIMO state space model? Imagine the plant was an aircraft, and I am attempting to model pitch and yaw control.
