Binary Mask of the noisy grayscale image?

HI, I need to make a binary mask for the object on 2D grayscale image. The problem is that there is a lot of noise and some parts of the object are barely visible. What method will be the best to obtain this binary mask? Additionaly this image is created by the 3d point of cloud. So maby better option is to make mask based on this cloud? If yes, whether somebody knows how to do it?

IF some parts of the region are barely visible, then probably a freehnad mask drawn by the user. See attached demo.

There is no one single best mask creation algorithm. The best option is the one that you say works the best. Now, how else can we help?
