Dimension issue state space model

I'm having an issue modeling this controller (something about the state-space output being incorrect in dimensionality). I've included my simulink model - please help!!

Hi Mary,

Could you provide further information on what exactly is the issue and what need to be done. The simulink model is working fine as per my understanding.
