How do I install and configure the network license manager for Polyspace products?

I have a concurrent license for my Polyspace products and I would like to setup the network license manager. How do I do so?

Polyspace R2013b and later:

With Polyspace R2013b, Polyspace now shares many core elements with MATLAB, including the license server. To install a Polyspace license server, refer to the MATLAB Network License Administration Documentation:

Polyspace R2013a and prior:

The Polyspace network license manager is not installed when you install your Polyspace products. To setup the license manager, you must do so manually. Please refer to Section 3 of the Polyspace License Installation Guide located on the Polyspace installation disk. This guide is contained in the following PDF document:

$CD\Docs\Install\Polyspace_License_Installation_Guide.pdf

(where $CD is the path the installation CD)
