how to fix error in port widths or dimensions?

When i run my model these two errors apper
The errors are in this part of the Model
The second port in the add block has a problem with dimensions don't know why. Also, the output signal of the Add block is supposed to be one dimentional, why does it show 3?

Can you share the model and inputs for the above model? That would help me out in reproducing the error.
