Automatic Extraction of ROI

I have this image as my input.
It's an ultrasound medical image of neck and arm portion and the marked portion is the root of the BP Nerve and I have to extract it without manually providing the coordinates.
Any help will be highly appreciated

Sorry, I don't know of any way. I don't see any distinguishing characteristics there that are markedly different than other places in the image. Maybe there is but I'm just not seeing it. Maybe try pattern recognition or deep learning. Or if you have a template (which I really doubt) then you can try normxcorr2().
