How to detect line segments oriented at certain angle?

I have some similarity matrixes which could be regarded as square figures(if necessary). I would like to know how can I extract the endpoints coordinates of all the line segments longer than 5 pixels and oritented 45 degree to both x and y axis (diagonal). Thank you!
